The association between long-term metformin use and low vitamin B12 levels has been proven. However, the prevalence estimates of metformin-induced vitamin B12 deficiency showed considerable variation among the studies. The potential of the deficiency to cause or worsen peripheral neuropathy in type-2 diabetes mellitus (T2DM) patients has been investigated with conflicting results. The aim of the study was to investigate: 1) the prevalence of vitamin B12 deficiency in T2DM patients on metformin; 2) the association between vitamin B12 and peripheral neuropathy; 3) and the risk factors for vitamin B12 deficiency in these patients. In this cross-sectional study, consecutive metformin-treated T2DM patients attending diabetes clinics of two public hospitals in South Africa were approached for participation. Participation included measuring vitamin B12 levels and assessing peripheral neuropathy using Neuropathy Total Symptom Score-6 (NTSS-6) questionnaire. The prevalence of vitamin B12 deficiency (defined by concentrations 6 were considered to have peripheral neuropathy. Photomicrographs of biopsy specimens from the paws of mice with cisplatin-induced neuropathy treated with control vehicle (left) or the HDAC6 inhibitor ACY-1083 (right) show that ACY-1083 reversed intraepidermal nerve fiber (IENF) loss, an early indicator of axonal pathology. Dashed lines indicate the basement membrane; arrows indicate IENFs crossing the basement membrane. Characterized by pain and loss of sensation in the hands and feet, chemotherapy-induced peripheral neuropathy (CIPN) can interfere with cancer patients’ treatment and significantly diminish their quality of life. The specimens were stained with antibodies for IENFs (red) and collagen (green). Researchers at The University of Texas MD Anderson Cancer Center are testing new treatments for CIPN, a serious adverse effect of chemotherapy and unfortunately one of the most common. “Neuropathy is a pretty pervasive problem; it can happen to patients with any type of cancer and be caused by many types of chemotherapy,” said Sarah Prinsloo, Ph. D., an assistant professor in the Department of Palliative, Rehabilitation, and Integrative Medicine. “And there aren’t a lot of effective treatments on the market for it.” Dr. In preclinical studies and clinical trials, she and her colleagues are testing pharmacological and noninvasive neuromodulation treatments to control—and perhaps even reverse—CIPN. A complex complication Chemotherapy induces neuropathy by damaging the nerves that control sensation and sometimes those that control mobility. To my knowledge, this is the first report of metformin-induced vitamin B12 deficiency causing neuropathy. Do you want to read the rest of this article? It is obtained primarily from animal sources such as meat and fish. Many breakfast cereals and nondairy beverages such as soymilk are fortified with vitamin B from animal foods enters the stomach it is protein bound and must be released by the action of pepsin in the stomach. It is then attached to R-proteins and is transported through the duodenum where it is hydrolyzed and free cobalamin is released . Cobalamin requires intrinsic factor (IF) to attach to it, in order to be absorbed in the terminal ileum. From November 2014 to May 2016, 40 patients with stage III colorectal cancer completed 12 cycles of FOLFOX-4 regimen. Twenty patients in the control arm received FOLFOX-4 regimen only, and 20 patients in the metformin arm, received the same regimen along with metformin 500 mg three times daily. The metformin efficacy was evaluated using National Cancer Institute Common Terminology Criteria for Adverse Events (NCI-CTCAE version 4.0), a12-item neurotoxicity questionnaire (Ntx-12) from the validated Functional Assessment of Cancer Therapy/Gynecologic Oncology Group and, the brief pain inventory short form “worst pain” item.
